Our home health program is designed to care for patients with specific medical needs and includes a broad array of professional and non-professional services. Working with the physician, a personal plan is designed to meet the needs of the client.

Skilled Nursing
Our Registered Nurses, Licensed Practical Nurses, and Vocational Nurses can assist with medical-related tasks and other complex healthcare needs. Your loved ones are provided with skilled nursing care that is tailored to their individualized needs. Among the services provided by our skilled nurses are:
- Wound care
- Cardiac pulmonary care
- Bowel and bladder program
- Diabetic teaching and care
- Anticoagulant monitoring
- Blood pressure monitoring
- Infusion therapies
- Tracheotomy care
- Ventilator care
- Catheter care
- Central line and port maintenance and management
Physical Therapy
Our Physical Therapists will work one-on-one with your loved ones so we can help them recover their body’s maximum function. Our PTs will perform an evaluation to determine the activities and treatment that will best yield successful outcomes. We utilize clinically proven treatment methods to ensure the health and safety of our clients.
Occupational Therapy
If your loved ones have emotional, developmental, social, or physical limitations, they can seek help from our Occupational Therapists. Through the therapeutic use of everyday occupations/activities, we can create intervention programs to help them improve, maintain, or recover their skills and quality of life.
Speech Therapy
Having communication and/or swallowing issues may hinder a person’s quality of life. Thus, we offer Speech Therapy to help, diagnose, and treat individuals with these conditions. We utilize individualized approaches to restore your loved one’s ability to communicate, swallow, and talk. With our intensive treatment plan, we can maximize your loved one’s quality of living.
Medical Social Services
Through Medical Social Services, we can help your loved ones find essential community resources and support that will aid in their recovery and quality of life. Once we determine their specific needs through comprehensive assessment, we will coordinate with healthcare providers, family members, and the community to arrange available resources; hence, providing the care they deserve.
Home Health Aide
Having a Certified Nursing Assitant (CNA) accompany your loved one will allow them to perform daily living activities with ease. This also allows family members or relatives to have peace of mind knowing that their loved ones are being looked after by well-trained professionals. HHAs assist with basic care and other healthcare tasks.
